Bowen’s Disease

Overview

  • Prevalence: Less common but as prevalent as SCC.
  • Common Locations: 75% occur on the leg.
  • Appearance:
    • Erythematous, well-circumscribed plaques ≥1cm.
    • Slightly raised with irregular hyperkeratosis.
    • Lacks the shiny surface and pearly rim of BCC.
    • May be bright red.

Risk Factors

  • Cancer Progression: SCC arises in 3% of cases.

Differential Diagnosis

  • Solar Keratosis
  • BCC: Shiny surface, pearly border, few dots of pigment.
  • Psoriasis: Silvery scale.
  • Eczema

Treatment

  • General Measures:
    • Excision.
    • Liquid nitrogen (more aggressive freeze than SK; may ulcerate on the leg).
    • Monitoring and watchful waiting.
