Grover disease

  • Also known as transient acantholytic dermatosis
  • cause of Grover disease is unclear 
  • Clinical
    1. produces small, firm
    2. intensely pruritic
    3. reddish-brown
    4. warty papules with minimal scale
    5. mainly on the upper trunk
    6. The rash can resolve over a few months, or persist for years.
  • It usually occurs in
    1. middle-aged to elderly men (typically 70-80 years).can occur same age women as well
    2. Trigger factors include
      1. Heat
      2. Sweating
      3. Fever
      4. occlusion, especially on photo-damaged skin. 
  • Differnetials
    1. Darier disease
    2. Miliaria
    3. Folliculitis such as malassezia folliculitis or gram-negative folliculitis
  • Diagnosis : by biopsy. 
  • Treatment
    1. can be difficult. 
    2. reduce heat and humidity and minimise 
    3. Apply a moderately potent topical corticosteroid to relieve symptoms
  • Treatment is to relieve the itch until spontaneous resolution occurs.
  • Effective treatments include
    • topical (preferable) steroids
    • oral corticosteroids
    • ultraviolet light.
